Basic information on health insurance for doctoral candidates of Lodz University of Technology

 

The principles of health insurance are regulated by the provisions of the Act of 27 August 2004 on health care benefits from public funds (Journal of Laws of 2019, item 1373, as amended). Doctoral candidates are subject to compulsory health insurance and have the right to use health care benefits free of charge in units that have concluded an agreement for the provision of health care benefits at the National Health Fund (Narodowy Fundusz Zdrowia, NFZ).

 

Doctoral candidates who are not Polish citizens

 

I. The doctoral candidate who is not a citizen of a member state of the European Union or a member state of EFTA and is not subject to compulsory health insurance in the Republic of Poland and in another member state of the European Union or a member state of EFTA, may get insured voluntarily by entering into an agreement with the NFZ if he is domiciled in the territory of the Republic of Poland.

 

The agreement is concluded on the basis of a written application and a certificate confirming the status of doctoral candidate and passport. A doctoral candidate who concludes the agreement and pays contributions in a timely manner, is entitled to receive a full range of health care benefits in Poland in units that have concluded an agreement for the provision of medical care with the NFZ. Within 7 days of signing the agreement, the doctoral candidate is required to register the agreement with the Social Insurance Institution (Zakład Ubezpieczeń Społecznych, ZUS) by submitting a ZUS ZZA form. The contribution specified in the agreement should be paid each month to the bank account specified by ZUS, not later than on the 15th day of the following month. The agreement is concluded for an indefinite period. The agreement is terminated on the basis of a written notice or after a month of uninterrupted arrears in the payment of contributions, or when the doctoral candidate gets entitled  to a compulsory health insurance, e.g. due to conclusion of an employment agreement. The document confirming the coverage for health insurance is a copy of the contract and proof of payment of the contribution for the last month. The entitlement to health insurance benefits begins on the day of the insurance coverage specified in the agreement and expires 30 days from the date of termination of health insurance at the NFZ.

 

II. The doctoral candidate who is not a citizen of a member state of the European Union or a member state of EFTA and is not subject to compulsory health insurance in the Republic of Poland and in another member state of the European Union or a member state of EFTA, recognized as a person of Polish origin within the meaning of the provisions on repatriation based on a decision of a consul or having a valid Polish Card, can get voluntarily insured by concluding an agreement with the NFZ if he is domiciled in the territory of the Republic of Poland. Health insurance contributions are paid by the University after the doctoral candidate has completed the Appendix 1 to the Announcement of Rektor of TUL on the procedure for payment of a doctoral scholarships and increase in doctoral scholarships in TUL IDS and submitted to the IDS TUL Office, 116 Żeromskiego Street, 90-924 room 210, building A33.

 

III. The doctoral candidate who is a citizen of a member state of the European Union or a member state of EFTA is entitled to free health care benefits provided by health care providers who have concluded agreements for the provision of health care benefits with the NFZ on the basis of the European Health Insurance Card or – if the doctoral candidate is not entitled  to insurance in the country of residence – he/she is registered for insurance by the University on the terms applicable to Polish doctoral candidates.
