on the organization and conduct of mandatory medical examinations of doctoral candidates of the Interdisciplinary Doctoral School of Lodz University of Technology
Pursuant to the Regulation of the Minister of Health of 26 August 2019 on medical examinations of candidates for secondary schools or universities and for qualifying vocational courses, pupils and students of these schools, students, students of qualifying vocational courses and doctoral students (Journal of Laws of 2019, item 1651). I establish as follows.
§1. General provisions
- Doctoral candidates at the Interdisciplinary Doctoral School of Lodz University of Technology, who are exposed to harmful, burdensome or dangerous factors during education, are required to obtain a medical certificate stating the lack of health contraindications to education at a doctoral school in a given discipline.
- The medical examinations referred to in section 1, are performed by occupational health units that have signed relevant agreements with the Marshal's Office of the Lodz Voivodeship in a given calendar year and are financed by the Local Government of the Lodz Voivodeship.
- The list of factors harmful, burdensome or dangerous to health is determined by the OHS Department Employee dealing with doctoral student matters in consultation with scientific supervisors/supervisor (s) or supervisor and auxiliary supervisor.
- The employee of the Occupational Health and Safety Department dealing with doctoral candidates updates the list referred to in § 1 (3) at least once a year.
- Doctoral candidates perform preliminary/periodic examinations in the field of occupational medicine and other mandatory examinations on the basis of referrals issued by the office of the Interdisciplinary Doctoral School of Lodz University of Technology (IDS TUL).
- Referrals are valid for 30 days from the date of issue. In the event of a justified random event that makes it impossible to complete the referral within the validity period, the referral will be re-issued.
- The scope of the medical examination and the date of the next examination are determined by the doctor, based on the data contained in the referral.
- The medical examination in the field of occupational medicine ends with the issuance of a medical certificate containing a statement on the existence or absence of contraindications to starting or continuing education at a doctoral school.
- Referral for a medical examination includes:
- designation of the teaching institution in the form of a print, sticker or stamp, containing its name (company), address, NIP and REGON;
- first name (s) and surname, date of birth and PESEL number of the examined person, and if it does not have a PESEL number - the type, series and number of the document confirming identity;
- information on the scope of training and on factors harmful, burdensome or dangerous to health, occurring during training at a doctoral school;
- date of issue of the referral;
- a stamp, print or sticker containing the name and surname of the person referring for a medical examination and their handwritten signature.
10. If the medical certificate states the existence of health contraindications to training at a doctoral school, the doctor shall inform the examined person about the reasons justifying the issuance of such a certificate and enter the justification in the medical examination documentation of the examined person. In this case, the decision to continue training in connection with the issued certificate is made by the Director of the Doctoral School, after consulting the scientific supervisor/supervisor or supervisors or the supervisor and auxiliary supervisor, provided that the medical contraindications found by the doctor do not prevent the implementation of training.
§2. Preliminary/periodic/follow-up examinations of doctoral candidates
- The doctoral candidate is obliged to receive a referral for preliminary medical examinations in the field of occupational medicine from the IDS TUL Office within 10 working days from the date of acquisition of the doctoral candidate's right, i.e. taking the oath.
- The doctoral student who is expiring the period of validity of the medical certificate is obliged
to perform periodic medical examinations in the field of occupational medicine, on the basis of a referral received from the IDS TUL Office. - The doctoral student who has been admitted to IDS TUL as a result of transfer from another doctoral school is obliged to perform preliminary medical examinations in the field of occupational medicine, on the basis of a referral received from the IDS TUL Office.
- The doctoral candidate resuming training at the doctoral school after returning from suspension of training lasting more than 30 days for health reasons is obliged to perform medical check-ups in the field of occupational medicine, on the basis of a referral received from the IDS TUL Office.
- The medical examination in the field of occupational medicine ends with the issuance of a medical certificate containing a statement on the existence or absence of contraindications to starting or continuing training at a doctoral school. The original medical certificate should be immediately submitted to the IDS TUL Office.
- Failure to deliver the above-mentioned decision to the IDS TYL Office within the prescribed period may constitute a basis for initiating disciplinary proceedings and applying disciplinary penalties specified in the Act – Law on Higher Education and Science, including suspension of
the right to a doctoral scholarship for a period of up to 1 year. - The template of the referral for preliminary/periodic/follow-up examinations referred to in §2 (1, 2, 3, 4) constitute Appendix 1 to this Announcement.
